Referral
Anyone—youth, partner agencies, or caring adults—can submit a quick online or phone referral. We respond within one business day to schedule a screening.
Initial Screening & Eligibility
A brief conversation (10–15 minutes) assessing level of housing instability, age (17–24), and immediate safety needs. If someone is outside our scope, we provide warm referrals to trusted partners.
Intake Appointment
At our Drop‑in Center or virtually, we complete consent forms, review rights & confidentiality, and map urgent needs (food, ID, health, housing). Each participant leaves with at least one tangible resource the same day.
Goal Setting
Program particpants choose priorities—like stable housing, employment, or mental health. Goals are specific, measurable, and youth‑led.
Ongoing Case Management
Dedicated Case Managers meet weekly or as needed to track progress, adjust goals, and celebrate wins. Services include housing search, benefits enrollment, job coaching, life‑skills workshops, and crisis support.
